Abstract: Objective To compare the efficiency of two methods of tooth extraction and the incidence of intraoperative and postoperative complications , as well as to provide basis for the new clinical therapies. Methods 132 cases were divided into two groups randomly, using surgaical impact air handpiece and traditional method respectively. The extraction time, intraoperative and postoperative complications of two groups were compared. Results The extraction time of experimental group was shorter than the control group and there was significant difference between the two groups（P＜0.05）. The incidences of root fracture, gingival laceration, and injury of temporomandibular joint of experimental group were lower than those in the control group, and the difference was statistically significant（P＜0.05）. There was no significant difference in the lingual bone plate fracture, tooth mobility, nurve injuries and tooth slip into fascial space in the two groups（P＞0.05）.The incidences of postoperative pain, limited mouth opening, postoperative swelling and dry socket in the experimental group were lower than those in the control group, and the difference was statistically significant （P＜0.05）. Conclusion The new method with surgical impact air handpiece can improve efficiency, as well as reduce the incidence of complications, and it's worth the clinical expansion.
