青少年单侧颞下颌关节盘移位患者颌骨三维变化的测量分析

Evaluation of the three dimensional changes in adolescent patients with temporomandibular joint disc displacement

摘要: 目的:分析伴有颞下颌关节盘移位青少年患者颌骨生长发育的变化及特点。方法:32例经MRI诊断为单侧颞下颌关节盘移位的青少年患者,采集头颅CT扫描数据并应用Surgicase 5.0软件进行三维头影测量。设定上/下颌骨标志点及参考平面,对水平向,垂直向及矢状向的生长进行测量,比较两侧颌骨发育的差异,并对测量数据进行统计学分析。结果:所有患者均表现出不同程度的面部不对称。下颌骨颏顶点(Gn)向关节盘移位侧偏斜,Gn与正中矢状面(SAG平面)的距离(Gn-SAG)为0.72~12.37mm,平均为4.72±3.41mm; 患侧下颌骨体部长度较健侧缩短(P<0.05);下颌角点(Go)、髁突点(Co)距SAG平面的离散(Go-SAG、Co-SAG) 均小于健侧(P<0.001);下颌中切牙中点(Li)向关节盘移位侧偏斜(3.44±2.72mm)较上颌中切牙点(Ui)的偏斜(1.19±0.63)增大(P<0.001);关节盘移位侧髁突内外径小于健侧,两者差异具有统计学意义(P<0.001)。结论:青少年期颞下颌关节盘移位与颌骨不对称畸形的发生有密切联系,且具有较为一致的特征。 关键词: 青少年 颞下颌关节盘 移位 三维

Abstract: Objective: To analyze the changes and characteristics of jaw growth in adolescent patients with temporomandibular joint disc displacement. Methods: 32 adolescent patients with unilateral temporomandibular joint disc displacement were involved in this study. CT scan were performed and the data was input in to Surgicase5.0 software for three-dimensional measurement. The upper/lower mandible markers and reference planes were set up to measure horizontal, vertical and sagittal growth, and to compare the difference between two sides of jaws. Statistical analysis was made by SPSS 16.0. Results: The development of the mandible in the side with TMJ disc displacement was smaller in all directions than those in the contralateral side. All of the patients showed mandibular deviation to affected side. Distance of point Gn to SAG plane ranged from 0.72 to 12.37mm, the average was 4.72 +/- 3.41mm; the ipsilateral mandibular body length was shortened compared to the normal side (P<0.05); the discrete distance of points Go and Co to SAG plane were shorter than those of the contralateral side (P<0.001); mandibular incisor point (Li) deviated to the side with disc displacement at a range of 3.44 +/- 2.72mm, overtook maxillary incisor point (Ui), with a date of 1.19 +/-0.63mm(P<0.001); transverse diameter of condyle was smaller in the side with disc displacement (P<0.001). Conclusion: Temporomandibular joint disc displacement in adolescence was closely related to the occurrence of asymmetric maxillofacial deformity and presented a consistent characteristic.
