汉族青年人上颌腭侧咀嚼黏膜厚度的CBCT研究

Assessment of the thickness of palatal masticatory mucosa in young people of Han nationality using Cone-beam computed tomography

摘要: [摘要]目的 应用锥形束计算机体层摄影术(cone beam computed tomography, CBCT)测量汉族牙周健康青年人上颌腭侧咀嚼黏膜厚度,并探讨性别、位置以及腭穹窿形态对上颌腭侧黏膜厚度的影响。方法 招募牙周健康的汉族青年志愿者32 例(男15 例,女17 例),使用CBCT图像分析法测量受试者上颌双侧尖牙至第二磨牙区正中距离龈缘3、6、9、12 mm水平处共 280个位点处的腭侧咀嚼黏膜厚度。并于上颌第一磨牙处计算每位受试者腭穹窿的宽高度比(W/H)得出中位数:2.59。由中位数2.59将受试者划分为高穹窿(W/H<2.59)和低穹窿(W/H≥2.59)组。结果 受测志愿者上颌腭侧咀嚼黏膜平均厚度为(3.66±1.15)mm,尖牙处上颌腭侧黏膜平均厚度为(3.23±0.71)mm,第一前磨牙处为(3.53±0.75)mm,第二前磨牙处为(4.87±0.58)mm,第一磨牙处为(3.21±0.90)mm,第二磨牙处为(4.39±1.67)mm。上颌腭侧黏膜厚度由尖牙区往后逐渐增厚,至第一磨牙区锐减,在第二磨牙区再次增厚且黏膜厚度从距龈缘向腭中缝处逐渐增厚(P<0.05);男性上颌腭侧咀嚼黏膜的厚度为(3.78±1.15)mm,女性上颌腭侧咀嚼黏膜厚度为(3.55±1.14)mm,男性组上颌腭侧黏膜厚于女性组(P<0.05);高穹窿组上颌腭侧咀嚼黏膜厚度为(3.72±1.07)mm,低穹窿组上颌腭侧咀嚼黏膜厚度为(3.60±1.21)mm。不同腭穹窿形态黏膜厚度存在差异(P<0.05)。结论 上颌腭侧咀嚼黏膜厚度从尖牙区向后逐渐增大,至第一磨牙区减小,在第二磨牙区再次增大,且各牙位区距龈缘越远处黏膜越厚。不同性别、不同腭穹窿形态者不同位点处的上颌腭侧黏膜厚度不同。建议选择上颌前磨牙区作为上颌腭侧移植瓣的主要供区。

Abstract: Abstract:Objective To use cone-beam computed tomography to measure the thickness of palatal masticatory mucosa in young people of Han nationality, analyze the change tendency and examine the associations of palatal masticatory mucosa with gender、palatal vault morphology and periodontal biotype. Methods 32 periodontally healthy youths of Han nationality (15 males, 17females) were included. The thickness of palatal masticatory mucosa, at five tooth positions from canine to second molar, were collected by CBCT from four different levels that 3, 6, 9 and 12mm away from the gingival margin. Futhermore, periodontal biotypes were determined according to transparency of periodontal probe through gingival margin in maxillary central incisor. Results The overall mean thickness of palatal masticatory mucosa was 3.66 1.15mm, 3.23 0.71mm in canine region, 3.53 0.75mm in the first premolar region, 4.87 0.58mm in the second premolar region, 3.21 0.90mm in the first molar region and 4.39 1.67mm in the second molar region. There were some differences in the thickness of palatal masticatory mucosa between the groups with high or low palatal vaults in some tooth positions. Conclusions The palatal masticatory mucosa thickness increased from canine to premolar region but decreased at the first molar region and increased again in the second molar region. And the palatal mucosa thickness also increased as the distance from the gingival margin increased. The association of palatal masticatory mucosa with palatal vault morphology was observed. The premolar region was recommended as the most appropriate donor site.
