不同镍钛器械预备根管后根尖偏移的CBCT比较研究

Cone-beam computed tomography analysis of the apical transportation after mechanical preparation with differentrotary nickel-titanium systems

摘要: 目的 通过CBCT测量镍钛器械预备根管后的根管偏移和中心定位率,来评价K3、Hero、MTwo根管预备的效果。方法 将按纳入标准收集的30颗离体牙随机分为3组,其中 A 组用 K3按冠向下法预备至#30/0.04; B组用 Hero预备至#30/0.04;C组用MTwo备至#30/0.04;然后,用锥束 CT 测量预备前后在距根尖3 mm处的近、远中的根管壁厚度,利用 Gambill 提出的公式计算根管偏移量和轴中心率,采用统计学比较3组的结果。结果 3组相比,无论是根管偏移量还是轴中心率,差异均无统计学意义(P>0.05) 。根管预备前后,3种镍钛对根管预备偏移量及轴中心率改变不明显(P>0.05)。结论 3种镍钛器械都能基本维持根管原来的形态,产生较小的根尖偏移量。

Abstract: Objective The aim of this study was to use high-resolution CBCT to evaluate the effects of three Ni-Ti rotary endodontic instruments-K3, Hero, and MTwo, on canal transportation and centring ratio. Methods 30 mesial roots of extracted mandibular molars were randomly divided into three groups with 15 teeth in each. The final instruments used were all #30/0.04.Teeth were scanned before and after instrumentation using CBCT to measure volume and shaping changes. Canal transportation and centring ratio were evaluated at 3 mm from the end of each root. The method developed by Gambill was chosen. Results No statistically significant differences were found among the three groups in terms of canal transportation or centring ratio at any level (P>0.05).These systems were able to produce centred preparations of canals with minimal transportation.Conclusions These three Ni-Ti rotary endodontic instruments can maintain the primary form of the root canal, and cause little canal transportation.
