正畸-正颌联合治疗的安氏Ⅲ类错合畸形与颈椎骨骼异常的相关性研究

A correlation study between cervical skeletal abnormalities and severe Angle class III malocclusion corrected with surgical methods

摘要: 目的 通过头影测量片检测正畸-正颌联合治疗的安氏Ⅲ类错合畸形与颈椎骨骼异常之间是否存在关联。方法 选取正畸-正颌联合治疗的安氏Ⅲ类实验组患者249名,其中男性120名,女性129名。无明显牙颌畸形的安氏I类对照组250名,其中男性85名,女性165名,所有患者年龄范围18-36岁。头颅侧位片记录前四个颈椎融合和后弓缺陷的数据。正畸-正颌联合治疗的安氏Ⅲ类错合畸形与颈椎骨骼异常之间的关系采用卡方检验进行分析。结果 实验组的颈椎融合发生率为50.2%,而对照组仅为13.2%,实验组显著高于对照组(P<0.001);实验组的寰椎后弓缺陷的发生率为4.8%,而对照组为2.8%,卡方检验显示两组间比较差异有统计学意义(P<0.05);实验组中有3.2%既有颈椎融合又有后弓缺陷,而对照组为1.2%,两组比较差异无统计学意义(P>0.05)。结论 正畸-正颌联合治疗的安氏Ⅲ类错合畸形患者颈椎骨骼异常的发生率明显增加。

Abstract: Objective To investigate whether there is an association between skeletal Class III and the cervical skeletal abnormalities by lateral cephalometric, panoramic radiographs, CBCT. Methods Select 249 patients with skeletal Class III who had accepted orthodontic-orthognathic surgery as the experimental group,120 were male and 129 were female;296 patients with Class I who had only accepted orthodontic treatment as the control group, 150 were male and 146 were female .All the patients’age were between 18-36.Record the data of the first four cervical fusion and posterior arch defects by the lateral cephalometric. The relationship between the abnormal cervical vertebra and skeletal Class III was analyzed by chi-square test. Results The occurrence rate of the cervical fusion in the experimental group was 71.84%(74 cases),while in the control group was 15.53%(16 cases).The occurrence rate in the experimental group was significantly higher than that of the control group(p<0.001);the occurrence rate of the posterior arch defects in the experimental group is 10.68%(11 cases),while in the control group was 4.85%(5 cases).Conclusions? There is an obviously increasing occurrence rate of the cervical skeletal abnormalities in patients with skeletal Class III.
