MBT技术结合微种植钉支抗治疗青少年骨性Ⅱ类高角错畸形的临床效果研究

摘要: 目的 探讨青少年骨性Ⅱ类高角病例通过拔除四颗前磨牙,使用MBT直丝弓矫治技术结合微种植钉支抗进行治疗,以达到改善面容的效果。方法 选择骨性Ⅱ类高角病例14例为研究对象,拔除四颗前磨牙,采用MBT直丝弓矫治技术配合微种植体支抗,对以上患者进行正畸治疗,并于治疗前后分别拍摄头影测量片,对测量项目数据进行统计学分析。结果 患者治疗后软组织侧貌改善,Z角增大,治疗前后差异具有统计学意义(P<0.05)。上下切牙唇倾度(U1-SN角度,L1-MP角度)减小,上下切牙相对上下颌骨内收(U1-NA距离、L1-NB距离),治疗前后差异具有统计学意义(P<0.05)。上下颌骨垂直关系控制良好,SN-MP角减小,治疗前后差异具有统计学意义(P<0.05)。结论 使用MBT直丝弓矫治技术结合微种植钉支抗通过拔除四颗前磨牙对青少年骨性Ⅱ类高角病例进行治疗,能控制上下颌骨垂直向距离,改善患者软组织侧貌,达到临床掩饰治疗的目的,降低患者正颌手术的可能。

Abstract: Objective To study the dentofacial changes of adolescent patients with skeletal classⅡhyperdiverge malocclusion after the extraction of four premolars using MBT appliance and mini-screw implant. Methods 14 randomly selected adolescent patients with skeletal classⅡhyperdiverge cases were treated by MBT appliance and mini-screw implant. In order to evaluate the effect of the treatment, the cephalometric radiographs were taken before and after the treatment, and the data were analyzed statistically. Results Patient’s facial profile had been modified. Z angle increased. There were significant differences before and after the operation (P<0.05). The angles of U1-SN and L1-MP decreased, while the distances of U1-NA and L1-MP adducted. There were significant differences before and after the operation (P<0.05). Vertical dimension control between the maxilla and mandible was successfully achieved. The angle of SN-MP decreased. There were significant differences before and after the operation (P<0.05). Conclusions Mini-screw anchorage and MBT appliance are effective in treating adolescent patients with skeletal classⅡ hyperdiverge cases, which can control the vertical distance between the maxilla and mandible, modify the soft tissue profile of patients, so as to achieve the goal of clinical hiding treatment and reduce the risk of orthognathic surgery.
