不同粘接方法对种植修复粘接剂残留影响的临床研究

Clinical study of the influence of different bonding methods on the adhesive residue of implant restoration

摘要: 目的:观察采用不同的粘接方法后种植冠周围粘接剂残留量,分析比较各种粘接方法的运用临床效果。方法:选取2018年6月~2019年6月在南昌大学第四附属医院口腔科种植的后牙区锥形锁柱种植体90颗,随机分为橡皮障组(A组)、粘接代型组(B组)和直接粘接组(C组),每组又根据冠边缘与牙龈的关系分为3个亚组,所有入选病例均在口内粘结后清理肉眼可见的多余粘接剂并拍X线片确认无粘接剂残留,随后取出基台和牙冠观察并记录粘接剂残留情况,再清理残留粘接剂后将基台冠就位于口内。结果:冠边缘位于龈下时,每组的不同亚组之间粘接剂残留量均有统计学差异(P<0.05);在冠边缘位于平龈水平时,三组之间粘接剂残留量无统计学差异;冠边缘在相同龈下深度时的粘接剂残留量对比,A组与B组之间均无统计学差异,但A组与C组、B组与C组间存在统计学差异。结论:冠边缘龈下深度会影响粘接剂残留,龈下越深粘接剂残留越多;当冠边缘位于龈下时,橡皮障法与粘接代型法均能有效减少粘接剂残留的量,采用粘接代型法操作更加方便且对减少粘接剂残留的效果更佳。

Abstract: Objective: Observe the adhesive residue around the implant restoration after using different bonding methods, to analyze and compare the clinical effects of various bonding methods. Methods: 90 locking-taper implants were implanted on the posterior teeth from June 2018 to June 2019 were randomly divided into rubber barrier group(group A), adhesive substitute group(group B) and direct adhesive group(group C), each group was divided into three subgroups according to the relationship between crown edge and gingiva. All selected cases were bonded in the mouth, remove the excess adhesive visible to the naked eye and X-ray was taken to confirm the absence of adhesive residue. The abutment and crown were then removed to observe and record the adhesive residue. Finally, the abutment crown was placed in the mouth after removing the residual adhesive. Results: The deeper the crown edge is under the gum, the more adhesive remains. There were statistically significant differences in adhesive residues between different subgroups of each group. When the crown edge was at the same subgingival depth, there was no significant difference in adhesive residues between group A and group B. There was no significant difference in adhesive residues between the three groups when the crown edge was at the level of the gingiva. Conclusion: The position of the crown edge would affect the adhesive residue, and the deeper the subgingival depth, the more adhesive residue. When the crown edge is located under the gingiva, both the rubber barrier method and the adhesive substitution method can effectively reduce the amount of adhesive residue, but the adhesive substitution method is more convenient to operate and has a better effect on reducing adhesive residue.
