骨性Ⅱ类青少年Twin-Block 矫治后气道和下颌骨变化的相关性研究

Correlations between the changes of upper airway and mandible after Twin-Block treatment in adolescents with ClassⅡ malocclusion

摘要: 目的 探讨 Twin-Block功能矫治器矫治后的安氏Ⅱ类下颌后缩青少年患者上气道变化与下颌骨变化之间的相关性。方法 选择使用Twin-Block 治疗安氏Ⅱ类下颌后缩青少年Forty例作为研究组,不矫治或仅戴用前庭盾的骨性Ⅱ类青少年20例作为对照组。研究组治疗前后拍摄 CBCT,对照组两次CBCT拍摄间隔1年左右。利用 Dolphin 软件对收集的数据进行三维重建,测量上气道及下颌骨研究指标,对两者变化量进行Pearson相关性分析。结果 下颌骨位置变化与上气道变化无明显相关关系(P>0.05),下颌骨长度(r=0.351)、下颌体长度(r=0.382)、前面高(r=0.355)及后面高(r=0.321)的变化量与LR-硬腭变化量显著正相关(P<0.05)。结论 Twin-Block功能矫治器在促进青少年的下颌骨生长发育的同时,增大腭咽横径,扩大上气道容积。

Abstract: Abstract: Objective to explore the correlations between the changes of upper-airway and mandible in adolescents patients with Class II division 1 malocclusion and mandibular retrusion treated with Twin-block functional appliance. Methods.40 cases of adolescent with Class II division 1 malocclusion and mandibular retrusion received Twin-block treatment were selected as the study group whereas 20 adolescents with skeletal Class II did not receive treatment or wearing vestibular shield were serve as the control group.The CBCT was taken before and after treatment in the study group while the two CBCT test of the control group were finished within 1 year approximately.The changes of upper-airway and mandibular were measured by using Dolphin software. SPSS 18.0 software package was used for Pearson correlation analysis. Results There was no significant correlation between the changes of mandible position and the upper airway (P > 0.05).The changes of mandibular length (r = 0.351), mandibular body length (r = 0.382), anterior facial height (r = 0.355) and posterior facial height (r = 0.321) were significantly correlated with the change of the lateral diameter of velopharynx(P < 0.05). Conclusion Twin-block appliance treatment can promote the growth and development of mandible and increase the lateral diameter of velopharyngeal to enlarge the volume of the upper airway in adolescent.
