成人安氏Ⅱ类与Ⅲ类错牙合 颅底形态差异的CBCT测量研究

Study of the cranial base morphology of patients with Angle class Ⅱ and Ⅲ malocclusion in adult through CBCT

摘要: [摘要] 目的 通过CBCT测量研究,探讨成人安氏Ⅱ类与Ⅲ类错牙合 畸形患者颅底形态的差异。 方法 选取19岁到26岁之间安氏Ⅱ类及Ⅲ类错牙合 畸形患者各20例,通过CBCT测量分析,比较两种错牙合 畸形患者颅底形态的差异。应用SPSS17.0 统计软件,对测量数据进行独立样本t检验及相关分析。结果 相对于安氏Ⅱ患者, 安氏Ⅲ类错牙合 患者颅底角(NSBa)显著减小,前颅底长、后颅底长及全颅底长变化无统计学差异。SNA 角与前颅底长(S-N)、后颅底长(S-Ba)及全颅底长(N-Ba)呈正相关。SNB 与颅底角(NSBa)呈显著负相关,与前颅底长(S-N)、后颅底长(S-Ba)及全颅底长(N-Ba)呈显著正相关。ANB 与颅底角(NSBa)呈显著负相关。结论 成人不同类型错牙合 畸形与颅底形态有关,颅底角对颅面形态影响较大。

Abstract: Abstract: Objective To investigate cranial base morphology of patients with Angle Class Ⅱ and Ⅲ malocclusion in the permanent dentition using tomographic cone beam (CBCT). Methods Forty patients aged from 19 to 26 years old were divided into Class Ⅱ and Class Ⅲ group according molar relationship. Linear and angular measurements were obtained in the mid-sagittal plane from the images of CBCT, and analyzed for all subjects. The mean differences between both groups were tested using Student’s t-test for independent samples. Pearson’s product-moment correlations were calculated between the parameters of the sagittal jaw relationship and cranial base. Results The cranial base angle deceased significantly in patients with Class Ⅲ malocclusion compared with Class Ⅱ malocclusion. Positive correlation was detected between SNA and S-N,S-Ba,N-Ba. The value of SNB was found significant negative correlation with NSBa,and positive correlation with S-N,S-Ba,N-Ba. There was also negative correlation between ANB angle NSBa . Conclusions There is association between malocclusion in sagittal jaw relationship and cranial base morphology in adult,and cranial base angle has more effects on maxillofacial morphology.
